Our hospitalists lead a team of nurses, physicians,
case managers and social workers who work
together to make a patient’s hospital stay
comfortable while addressing medical and
administrative needs.

Responsibilities of a hospitalist include:
• Coordinating a patient’s care.
• Communicating with primary care physicians
and other doctors involved in a patient’s care.
• Ensuring patients receive the necessary
instructions when they leave the hospital.
• Forwarding appropriate records and findings
from the hospitalization to the patient’s
primary care physician.
Going Home
Your hospitalist will facilitate your discharge by
sending your primary care physician a summary
of your hospital care, any records, post-discharge
plans and medications.
After you have been discharged, make sure to call
for a follow-up appointment with your primary
care physician. Take with you any new or changed
medications, and any other new orders for diet,
therapy or follow-up care.
Billing
The billing process for hospitalist care is similar
to being billed by a primary care physician for a

What is a Hospitalist?
A hospitalist is a physician who specializes in
caring for patients in the hospital. Hospitalists
specialize in internal medicine or family medicine
and draw from a wide range of medical experience.
Coordinated Medical Care
After you are admitted to the hospital, a member
of our hospitalist program will coordinate and
administer any tests and treatments you may
need during your hospital stay.
To ensure seamless care, your primary care
physician will share your health records and
information with one of our hospitalists.
This information includes your current health
condition, medical history and treatment
preferences.
Hospitalists are available on-site 24 hours a day.
They are accessible in emergencies or when
questions arise. They are familiar with hospital
procedures and processes, which allows them to
function efficiently in ordering tests, consulting
with other specialists, and adjusting treatments.
Hospitalists often care for patients of local
physicians and medical groups, as well as patients
who do not have a primary care physician and
transfer care of the patient back to the primary
care physician upon discharge.
Depending on the length of stay, patients may be
under the care of more than one hospitalist.
