Duke Endowment Awards Grant to AccessHealth

Greenwood/SC: Self Regional Healthcare has received a $500,000 grant from The Duke Endowment to expand a community network of care for chronically ill, low-income, uninsured individuals through the AccessHealth Lakelands program.

Self Regional Healthcare’s AccessHealth Lakelands program serves qualifying individuals in the Lakelands region. Services include case management by certified Community Health Workers, establishment and coordination of care with a primary care home and assistance with other barriers to obtaining optimal health. This program partners with local and state agencies to provide resources to allow participants to take control of their health and well being. To date, AccessHealth Lakelands has assisted over 800 clients. The Endowment grant will be used to continue to fund existing services as well as reach more people within the communities we serve.

Based in Charlotte and established in 1924 by industrialist and philanthropist James B. Duke, The Duke Endowment is a private foundation that strengthens communities in North Carolina and South Carolina by nurturing children, promoting health, educating minds and enriching spirits. Since its founding, it has distributed more than $3 billion in grants. The Endowment shares a name with Duke University and Duke Energy, but all are separate organizations.
