Facility Screening Questions

1. Have you recently tested positive for SARS-COV-2?
2. Do you have symptoms of COVID-19 (fever, chills, cough or shortness of breath)?
3. Have you had close contact with someone with the SARS-COV-2 infection?
If you answer yes to any of these, please do NOT enter our facility.

Self Medical Center entry hours are
8 a.m.-7 p.m., 7 days a week.

Visitation Policy in this facility

Beginning Friday, December 29 Self Regional Healthcare will be requiring all providers and team members to wear masks in all clinical areas throughout the organization, and all patients and visitors will be highly encouraged to wear masks inside SRH facilities to help prevent further spread. We also will be limiting visitation in our ECC to 1 visitor per adult patient, and 2 visitors per patient who is 15 years old and younger. At this time, South Carolina is experiencing some of the highest rate of respiratory illness in the country, so we also strongly recommend that all individuals be vaccinated for Flu, Covid-19 and RSV. After 7 p.m., please use the Emergency entrance only.
