We offer the following benefits to all Full Time, Part Time employees who work at least 32 hours per pay period, and Self Plan employees.

- Outpatient services provided at Self Regional Healthcare, The Outpatient Surgery Center, and Self Regional Healthcare owned practices are paid at 100% after a $100 deductible. The $100 deductible applies towards the plan year deductible not to exceed the plan year deductible.
- USHS Physician Office Co-pay of $20, Express Care co-pay $25, Emergency Room co-pay $75
- Covered Services include:
- Speech & Hearing, OT, PT
- Well Child Visits & Immunizations up to 2 years
- Well Care Benefit after age 2 – up to $250 per covered person per plan year (in-network only)
- Hospitalization
- Prescriptions
- Utilization Review
Medical Deductibles:
- Plan 1 -- $325 individual deductible; $975 family deductible
- Plan 2 -- $525 individual deductible; $1575 family deductible
Prescription Benefit:
- Self Regional outpatient pharmacy
- Hospital cost plus 10%
- Prescription drug card - $100 single deductible – up to $300 family deductible
- Retail pharmacies in network -- 10% co-pay for generic drugs, 20% co-pay for preferred brand name drugs and 40% co-pay for non-preferred brand name drugs
Physician Office Visit:
- $20 co-payment (this covers the physician charge only). Ancillary charges such as lab work, injections, x-rays, are paid at 80% after the deductible. However, outpatient services provide at Self Regional Healthcare, The Outpatient Surgery Center and Self Regional Healthcare owned practices are paid at 100% after a $100 deductible. The $100 deductible applies towards the plan year deductible not to exceed the plan year deductible.
Dental Insurance:
- Diagnostic/Preventive services covered at 100%/Basic and Periodontal services covered at 80%
- Prosthodontics covered at 60%
- $25 individual deductible; $50 family deductible
- Orthodontic maximum of $1,000 lifetime/Benefit year maximum of $1,000 for Basic and Prosthodontics
Life Insurance:
- Plan A – 1 x your annual earnings up to a maximum of $500,000 (SRH pays for this benefit).
- Plan B – you may choose to pay an additional 1 x your annual earnings up to a maximum of $500,000.
- Plan C – you may choose to pay an additional 2 x your annual earnings up to a maximum of $500,000.
- Dependent Life:
- Spouse - $2,000, $5,000 or $10,000
- Each child (7 days to 6 months) - $1,000 / Each child (6 months to 19 years or up to age 26 if Full Time student) - $2,000, $5,000 or $10,000
*Long Term Disability:
- Plan A – Provides long term disability benefit equal to 40% of basic monthly earnings. (This plan is provided by Self Regional Healthcare at NO cost to you.)
- Plan B – Provides long term disability benefits equal to 60% of basic monthly earnings. If this optional benefit is chosen, you will still receive the long term disability benefit equal to 40% of basic monthly earnings that plan A provides. You will pay a bi-weekly premium to receive an additional 20% of your basic monthly earnings.
- Maximum monthly benefit is $7000.00.
Flexible Spending Account:
- Health Care Spending Account – You may deposit up to $5,000 per plan year for out of pocket expenses regarding medical, dental, prescriptions, vision care, etc. HCSA may not be used for Cosmetic procedures.
- Dependent Care Spending Account – You may deposit up to $5,000 per year for child or adult (at home or day care center, care at adult care centers, etc.)
Tax Sheltered Annuity 403(b) Plan:
- All Full Time, Self Plan, and Part Time employees who work at least 32 hours per pay period are eligible to participate in the plan following their employment date. Self Regional matches dollar for dollar up to 3% of salary. Employee contributions are pre-tax deductions. Resource employees can contribute but are not eligible for employer match.
Deferred Compensation 457(b) Plan:
- All Full Time, Self Plan, and Part Time employees who work at least 32 hours per pay period are eligible to participate following their employment date. Employees must be scheduled to contribute the maximum allowable amount in the 403(b) before contributing to 457(b). Employee contributions are pre-tax deductions.
*Voluntary Short Term Disability Benefit:
- You may choose from a 7 day or 29 day elimination period.
Voluntary Cancer Insurance Benefit:
- Choice of Basic or Enhanced Coverage
Voluntary Optional Life Insurance Benefit:
- Individual policy with age-rated guarantee issue
Voluntary Vision Plan:
- Co-Pays $0 or $10; $200 allowance on eye wear and contact lenses every 12 months
Cafeteria Plan Benefits:
- All employee paid premiums are paid with pre-tax dollars with the exception of STD, LTD, & outside life (universal life) and Dependent Life.
