MyChart is a patient portal that offers secure on-line access to portions of your medical records and health at Self Regional Healthcare. MyChart is a free service offered to our patients available at https://mychart.selfregional.org.

Patients who wish to participate will be issued a MyChart activation code during their clinic visit or via an activation email. You can also signup for a MyChart account directly without an activation code. Click your option below:

You might have heard about MyChart from your health care provider. MyChart is a great way for you to see information about your medical care and communicate with your care team. I’ll show you how easy it is to sign up for a MyChart account. You’ll see how to do this on a mobile device, but the steps are similar from your computer. Chances are you’ll be granted access by your health care provider before, during, or after an appointment. However, you could create an account on your own on MyChart.org. From there, you’ll find information about how to establish an account with your health care organization. You might be able to create an account right away, or you might need to request approval, depending on their policy. Let’s say you do get an email from your provider after an appointment with a link to sign up for MyChart. Simply open the link, and you’re taken right to the sign-up page to confirm your identity, typically by entering your birth date. Now that MyChart knows that you’re you, you can choose to create a MyChart Central account. When you have MyChart accounts at all the places you get care, MyChart lets you use a single login to see them all in one spot. This will take a few extra steps right now, but we’ll make logging in faster next time. Click here to set up your MyChart Central account. Then, review and agree to the terms and conditions of using MyChart Central. Associate your mobile number with your account using a verification code MyChart sends to your phone. When you get the code on your phone, enter it here. Now follow similar steps to associate your email with your account. The email address you verify will be your username. Here, set up your MyChart central password. To keep MyChart accounts in sync across all the places you get care, keep this checkbox selected. This way, if you change important information like a mailing address, all your accounts automatically get the update. Now, you can see information from the healthcare provider who sent you the activation email. You already reviewed and accepted MyChart Central’s terms and conditions, but your health care provider might require you to review and accept theirs too. Because it’s your first time accessing your account, there’s a short video and tutorial for you to watch, if you’d like. I’ll choose to skip it for now. And with that, your brand new MyChart account appears. The main view of MyChart is your health feed, which shows the most important alerts and information for your care. eCheck-In For All ( Mobile )

Checking in before your visit lets you document information relevant to the visit and speeds up your time at the front desk. eCheck-in allows you to virtually complete this process in MyChart, whether or not you have a MyChart account. Information you provided during previous visits or scheduling is saved, so you don’t have to enter it more than once. Let’s say that you have an appointment later today, and you don’t have a MyChart account. You receive an email notification with a reminder to complete eCheck-in. If you did have a MyChart account, you’d see the notification to complete eCheck-in from your health feed instead. In this case, in the email, I’ll tap Check-in online. First, provide or verify your demographic information. This helps your health care provider verify your identity and visit information. Enter your Date of Birth and any other required information. Then, tap Verify. Now, make sure your contact information is correct so you can receive further communications about this visit. Verify that your personal details are also correct. The scheduler collected your insurance information when you made the appointment, so you just need to verify it. If you need to update or add a new coverage, you can also do that here. If there’s a copay for the visit, you can pay that now. Now confirm or add your payment method. Review the medication list to verify your current medications. In this case, the list doesn’t include the ibuprofen you’ve been taking for back pain. I’ll add that now. Select or add your preferred pharmacy, confirm your current medications, and tap “Next.” Next, you’ll likely be asked a series of questions specific to your reason for visit. Your care team can see your responses, so you don’t have to spend time answering common questions during your visit. I won’t demonstrate answering all of these questions now, but I’ll show you how it looks filled out so you get the idea. When you’re done answering all the required questions, Finally, review and sign the required consents. You can use your finger or stylus to sign the form, or you can accept the signature that was auto-generated for you. Once you’ve signed, tap to accept the form, and then tap to submit. You’ve completed eCheck-in. The front desk and clinical staff will have all of this information on file to reference during your visit. To help manage future appointments online and make use of other features, you can sign up for a MyChart account here as well. Now that you’ve completed your pre-visit tasks, your healthcare provider can use your mobile device location to automatically mark you as arrived. Thanks for watching! Common MyChart Features

Listed are some of the most requested and common MyChart Features.

View Your Health Summary

The MyChart Health Summary provides patients with a summary of their clinical information. It includes up-to-date information about current health issues, medications, allergies, immunizations and preventive care.

  • Select Your Menu in MyChart and click on Health Summary under the My Record topic
  • Select the tab to view current health issues, medication, allergies, immunization/vaccinations and preventive care.

Access your Test Results

The MyChart Test Results activity provides lab and screening results for the patient. Patients can view graphs and trends of their results for past years.

  • Select the Test Results option from the MyChart header menu or from Your Menu under the My Record topic.
  • Click on an individual test result to view. Select View Trends to see your results over time.
  • From the Results Trends activity, select Download to save a PDF table of your past results.

Request Prescription Refills

The MyChart Medications page allows patients to review their medications and information about them, including the start date and instructions for each prescription.

  • Click on Medications from the MyChart header menu or from Your Menu under the My Record topic.
  • Select Request refill to send a refill request to your provider for approval. (Consult with your Provider for any medications you are unable to renew via MyChart.)

Proxy Access for family members

With proxy access, parents can access their children’s MyChart accounts, a husband or wife can access a spouse’s MyChart account, or caregivers can be granted access to the MyChart accounts of those for whom they care.

Proxy Access for a Minor’s Record

  • From Your Menu in MyChart, select Request Access to Minor’s Record under the Sharing topic.
  • Complete the form with the minor’s information and click Submit Request.
  • Support/Clinical Staff will process your request and send a response when completed.

Proxy Access for an Adult Record

  • From Your Menu in MyChart, select Sharing Hub under the Sharing topic.
  • Click the option, A family member, close friend or caretaker.
  • Click Manage ongoing access to your MyChart account.
  • Click Invite Someone.
  • Complete the form with the name and email address of the person you wish to share your MyChart account with and click Send invite.

Link Your MyChart Accounts

Patients can use MyChart to view their health information from other organizations. When you connect your accounts from the participating organizations, you will be able to see your clinical information all right here in your SRH MyChart account.

  • From Your Menu, select Link My Accounts under the Sharing topic.
  • Under the Discover tab, select from the Organization in your area list or type in the name of the organization in the field.
  • Expand the arrow, if needed and click on Link account. (You may be required to complete verification or login requirements to link your accounts.)

Paying Your Bill

The MyChart Billing Summary activity allows patients to view account information for billing accounts for multiple guarantors. Patients are able to view balance details, payment information, communications such as statements or letters, and pay on any outstanding balances. Payment Plans and Financial Assistance may be available for qualifying patient accounts.

  • From Your Menu, select Billing Summary under the Billing topic.
  • Click on View Account Details or View Balance Details on the guarantor billing card.
  • Several billing tabs are available:
    • Overview: A summary for the current guarantor account.
    • Details: A breakdown of your visit account charges and adjustments.
    • Payments: A listing of payments made on your account.
    • Communications: Billing statements or letters pertaining to your account.

Messaging A Provider

Patients can send messages using the Messages activity in MyChart. Messages can be sent for non-emergent medical advice to your Primary Care Provider/Care Team or customer service questions ranging from medical records to billing to website issues. Patients can also view automated messages for scheduled appointments, appointment reminders or modified appointments.

  • From Your Menu, select Messages under the Communication topic.
  • Select Send a message to start a conversation. Choose your message option under What would you like to do?. Messages require a subject (*Subject) and body (*Enter your message) to send a message of any type. Attachments can be included by using the Attach option.
  • Click Send once your message is completed and please allow a few days for staff to repond.

Hello World Text Messages

Hello World, Epic’s enterprise communication service platform, is our new SMS gateway used for sending text messages to patients. With Hello World’s SMS gateway, we will keep patients, proxies, and guarantors engaged by communicating important information through text messages.

How do I start receiving text messages?

Text START to 45314 to opt in to receive text messages from Self Regional Healthcare.

How do I stop receiving text messages?

Text STOP to 45314 to opt out of receiving text messages from Self Regional Healthcare.

You may also opt out of receiving text messages via MyChart. Log into MyChart. From the MyChart menu, select Communication Preferences under the Account Settings topic. Below the General section, select Manage your subscription. On the Manage Text Subscriptions screen, toggle off the Message Subscription option.

How do I update my mobile number for text messages?

To update your mobile number for text messages, log into MyChart.

From the MyChart menu, select Personal Information under the Account Settings topic. Click Edit under Contact  Information, update your mobile phone number and click Save Changes.

I’m receiving some messages, but not all?

To update the types of notifications you receive via text message, log into MyChart.

From the MyChart menu, select Communication Preferences under the Account Settings topic. Select the Text Message option under Settings. Under Details, expand the Notification Topic and select the method of communication by toggling the switch On or Off. Click Save Changes.

MyChart Two-Step Verification

Patients can ensure that their account stays secure even if someone else has their username or password by turning on two-step verification. When this feature is turned on, patients must enter a code that is sent by email or text message to log in to MyChart, in addition to using their username and password.

When patients log in to MyChart, they will be prompted to verify their email address and phone number to enable two-step verification. MyChart Two-Step Verification is now required to log into your MyChart Account.

Two-Step Verification (Logging in)

After logging into MyChart, patients will be REQUIRED to setup two-step verification.

At this prompt, patients options are listed below:

  • Set up two-step verification: Proceed with setting up the two-step verification feature.
  • Cancel and log out: Logs the patient out of MyChart immediately.

Two-factor authentication can no longer be turned off in MyChart. Patients are able to bypass entering the passcode on known/saved devices by checking the Trust This Device option when entering their code.
