Our clinic focuses on making sure your medications are working safely and effectively for you. Your care team will review all of your prescriptions, check for possible interactions, and ensure each medication is appropriate for your needs. If needed, adjustments can be made to improve results and reduce side effects.
Medication management can be especially helpful for patients with conditions like diabetes, heart disease, high blood pressure, or high cholesterol, as well as anyone taking several medications or having trouble staying on track with their treatment plan.
During your visit, you can expect a thorough review of your medications, clear guidance on how and when to take them, and answers to any questions you may have. Ongoing support and follow up help ensure your treatment continues to work well for you over time.

- All we need is a referral from your primary care provider or specialist. A member of our staff will reach out to make an appointment with you.
What are the benefits?
- As part of our patient visit, the clinical pharmacist will review your benefits plan formulary, available assistance options, and equivalent medication alternatives to establish a comprehensive plan. We complete prior authorization requests, and enrollment in patient assistance programs. Most patients’ prescription costs will be reduced by programs offered at the SRH Medication Management Clinic and SRH Outpatient Pharmacy.
- The addition of a clinical pharmacist to the patient care team adds an additional resource for questions and patient education.
- The Medication Management Clinical Pharmacist does not replace the referring provider. Referred patients continue to see their provider as usual. As part of a patient’s care team, a clinical pharmacist works collaboratively with providers to optimize medication management through a Collaborate Practice Agreement (CPA).
- The role of the clinical pharmacist is to: promote appropriate drug selection, evaluate cost, teach proper self-injection technique and administration of medications, monitor and evaluate drug efficacy, and optimize dosing. This reduces risk of drug interactions, medication errors, and adverse events. We have the time to answer your questions, and we make appropriate changes in medication therapies when needed in collaboration with your primary doctor.
